The State Board of Local and Regional Jails has approved a new Serious Incident Report Form that replaces the previously used Report of Inmate Death form. The action was taken at the Board’s November 19, 2025, meeting. The updated form is intended to provide a single reporting document for a range of serious events in local and regional jails. Incidents covered by the form include:

  • Firearm discharges
  • Erroneous releases
  • Escapes
  • Fires with evacuations
  • Hostage situations
  • Recaptures of escapees
  • Deaths

An earlier version of the form was distributed with a draft watermark. Sheriffs should discard that version and use the updated Serious Incident Report Form going forward.
